Responsibilities

  • Deliver comprehensive personal care, assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary.
  • Monitor patients’ vital signs and observe for any changes in health status or behavior, documenting findings accurately while respecting HIPAA privacy standards.
  • Prepare nutritious meals tailored to individual dietary needs, ensuring safe food handling and cleanliness in accordance with health regulations.
  • Administer medications as prescribed, following care plans meticulously while maintaining compliance with all safety protocols.
  • Support residents with cognitive impairments by engaging in memory care activities and providing specialized Alzheimer’s or dementia care to promote comfort and safety.
  • Maintain a clean and safe environment by performing regular cleaning duties and adhering to infection control procedures.
  • Collaborate with healthcare professionals and family members to develop and update personalized care plans that reflect each individual’s unique needs and goals.

Qualifications

  • Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care environments is highly preferred.
  • Certification in first aid and CPR is required; additional training in HIPAA compliance, hospice care, or social work is a plus.
  • Demonstrated ability to work with individuals with developmental disabilities, autism spectrum disorder, or memory-related conditions such as Alzheimer’s disease.
  • Strong patient observation skills for monitoring health status and recognizing urgent issues promptly.
  • Physical ability to perform heavy lifting and assist residents with mobility using equipment like Hoyer lifts or resident’s <PERSON>.
  • Excellent communication skills for collaborating effectively with team members, residents, and families.
